Salivary cortisol concentration in the intensive care population: correlation with plasma cortisol values

Increasing interest has recently been focused on the measurement of plasma cortisol levels in intensive care patients, both for prognosis and targeting therapy1,2. There are, however, potential errors in this approach. Cortisol in plasma is approximately 95% protein bound to cortisol binding globulin and albumin, but its biological activity is mediated only by the unbound fraction. Changes in plasma protein levels, which are commonly seen in intensive care patients, may not change the total measured plasma cortisol levels, but the effect on free cortisol is uncertain3,4.
