A 21st Century View: The Days of Cheap Health Insurance Are Gone

I have been observing the crisis in individual and group health insurance, and while general reasons for the problem and its cure have been reported, the specific reasons and specific solutions have not.

I believe readers deserve a wider view of the problem before they can appreciate the steps needed to correct it. I propose a look at the problem from the inside out.
