CHICAGO--Jul. 30--The "managed care" industry faces a conundrum: People say they dislike managed care even as they join health maintenance organizations by the millions.

"I wish I didn't have to use the words 'managed care,' " said Dr. John Ludden of Harvard Pilgrim Health Care, which has more than 1 million members. "There's something about the term that turns people off."

"Managed care," the catch-all label for any kind of health care in which physicians or insurers direct medical care to contain costs, is well on its way to replacing "fee-for-service" ...
