A PRESTON surveyor killed in a 20ft fall at a disused church fell through a trapdoor covered by plywood, health and safety bosses have revealed.
David Humphries, 64, fell to his death while inspecting the upper level of Holy Trinity Church in Bolton.
Mr Humphries was in the church with a colleague when the accident happened on Monday, May 15.
His firm, David Humphries and Associates, is based on Garstang Road, Preston and he lived in Woolton Park, Liverpool.
