Using appropriate abbreviations in perioperative documentation.(CLINICAL ISSUES)

QUESTION: I graduated from nursing school more than 20 years ago and pride myself on staying up-to-date on current practice trends. As a nursing student, I was taught many abbreviations related to medications. Now, these abbreviations are considered risks to patient safety. How can practice be changed after all these years, and what tools are available to assist with the change?
