DURING THE TWENTIETH CENTURY, mortality in the Western world and in Japan declined to unprecedented levels (Coale, 2003; Preston, 1976; Tuljapurkar, Li and Boe, 2000; White, 2002; White and Preston, 1996; Wilmoth, 1998; Riley, 2001; Vallin and Mesle, 2005).
