Perhaps the biggest problem with all the suggestions on how to improve our country's health care system is that few people recognize that we don't have a health care system! What we have is a hodge-podge of independent providers of many different health services, often overlapping in what they provide, frequently not communicating with each other, and rarely considering the costs of the services they provide.
