Healthcare rationing has long been a third-rail issue for U.S. health care. Poll-sensitive politicians and fainthearted academics alike have conspicuously avoided touching that rail, unwilling to openly suggest that the nation may require some type of rationing to preserve its healthcare system.

Yet many healthcare policy leaders are now acknowledging (if only among themselves) that American health care is almost certainly headed for such a system.
