During the 1970s and 1980s, quality assurance (QA) was a provider-oriented, defensive response to requirements of external agencies, such as the Joint Commission on Accreditation of Healthcare Organizations, [1,2] and a reaction to increased liability of health care centers for the performance of all individuals, including physicians.
